Ask the client for

  • W-2s from every employer

  • 1099 forms (interest, dividends, brokerage, freelance income)

    1099-INT, 1099-DIV, 1099-B, 1099-NEC or 1099-K, and any K-1s from partnerships or trusts.

  • Form 1098 for mortgage interest

  • Property tax bills paid

  • Receipts for charitable donations

  • Childcare costs and the provider's tax ID

  • Form 1095-A, if insured through the Marketplace

  • Estimated tax payments made during the year

    Dates and amounts for federal and state.

  • Last year's tax return

    Only for new clients.

Tips

  • Repeat it yearly on January 15 so the list is waiting as forms start to arrive.
  • Use the notes to list last year's forms so the assistant can spot a missing 1099.
